  1. Open the application in your browser by clicking the PDF icon to the left.
  2. Our application is a fill-in PDF that will allow you to fill out the grant application on your computer. You can enter your responses by clicking on the applications fill-in boxes with your cursor, and typing your response to each question. Check boxes can be checked by clicking on them. You can also print the application, and fill it out by hand if you prefer. Please answer all questions in the application before submitting it."
  3. After you complete the application there are two ways you can submit it to us. The standard way is to print it, and mail it to our address below:
